A clinical breast exam (CBE) is a physical examination of the breasts performed by a trained physician or healthcare provider. Using a systematic pattern of gentle pressing, your physician examines both breasts, the nipple area, and the lymph nodes under your arms and along your collarbone.
The purpose is to identify lumps or masses, changes in texture (like an orange peel), nipple changes (inversion or discharge), skin changes (redness or dimpling), and lymph node enlargement. It complements mammography and self-exams — each method detects what the others might miss.
No special preparation is required. The entire exam typically takes 5–10 minutes and should not be painful. Here is a breakdown of what happens during your visit.
Your physician visually examines both breasts with your arms at your sides, then raised above your head. They look for asymmetry, swelling, and skin changes.
While lying down, your physician uses overlapping circular motions to systematically examine the entire breast — from collarbone to rib cage, and armpit to breastbone.
Your physician gently checks the nipple for any discharge by applying light pressure. Any unusual fluid is flagged for further laboratory evaluation.
Your physician carefully feels the lymph nodes under each arm and along your collarbone. Enlarged nodes can indicate infection, inflammation, or other concerns.

Using the pads of your fingers, move them around your entire breast in a circular pattern from the outside to the center, checking the entire breast and armpit area. Check both breasts each month.
Visually inspect your breasts with your arms at your sides, then with your arms raised, and then with your hands pressed on your hips. Look for any changes in contour, swelling, dimpling, or nipple changes.
Place your arm behind your head. Apply light, medium, and firm pressure in small circular motions covering the full breast. Perform self-exams at the same time each month — ideally a few days after your period ends when breasts are least tender.
